If you are making a referral…
YSB receives many referrals from area schools, court services, police, DCFS, and other local service providers. If you would like to make a referral please download the General Referral Packet FY12 and fax it to 815.748.2019, attention Andrea Broska.

Local Area Network 33 – DeKalb County Wraparound Planning

Purpose
The purpose of wraparound planning is to assist children who are at risk of or are exhibiting poor attendance, truancy, suspension or expulsion from school, utilizing natural and community service supports.  Wraparound planning is a strengths-based process used to promote and support educational services for children.

Eligibility Criteria
Any child, ages 3 – 21, with emotional, behavioral or academic issues that put them at risk of or has resulted in any of the following: poor attendance, truancy, suspension or expulsion from school.

How it Works
The basic philosophy of wraparound planning is to use the strengths of the family and community together with ‘thinking outside of the box’ to develop creative solutions to assist individual families.  Child and Family Teams look at multiple ‘Life Domains’ to determine the strengths a family possesses and what services/goods are needed to help the child succeed in school. Plans should include community services/goods that are available to the family through other funding sources as well as services/goods requested through flexible dollars from the LAN.  Whenever possible, natural supports will be utilized to provide assistance.
